Your doctor says your BMI is 26. That puts you in the overweight category. But you lift weights four days a week, carry visible abs, and have a resting heart rate of 52. Something does not add up.

It does not add up because BMI was never designed to measure individual health. It was designed for population-level statistics, and it shows when you apply it to individuals with anything other than an average body composition.

What Is BMI and Where Did It Come From?

Body Mass Index is a ratio of weight to height squared. The formula is:

BMI = weight (kg) / height (m)^2

The categories are straightforward:

  • Under 18.5: Underweight
  • 18.5 to 24.9: Normal weight
  • 25 to 29.9: Overweight
  • 30 or above: Obese

A Belgian astronomer and statistician named Adolphe Quetelet developed the formula in the 1830s. He was not a physician. He was looking for a way to describe the average build of populations, not to diagnose individuals. The formula stuck because it is cheap, fast, and requires no equipment beyond a scale and a tape measure.

What Is Body Fat Percentage?

Body fat percentage measures the proportion of your total weight that is fat mass versus lean mass (muscle, bone, organs, water). It is a direct measure of adiposity rather than a proxy for it.

The measurement methods range from cheap and approximate to expensive and precise:

  • Bioelectrical Impedance Analysis (BIA): Found in many smart scales and gym InBody devices. Sends a small electrical current through the body and estimates fat based on resistance. Accuracy varies, but modern multi-frequency devices correlate well with DXA (r = 0.83 to 0.93 for body fat percentage).
  • Skinfold calipers: Cheap and reasonably accurate in trained hands. Requires measuring skin folds at 3 to 7 sites. Error rate of 3% to 5% depending on the technician.
  • DXA (Dual-Energy X-ray Absorptiometry): The gold standard. Uses low-dose X-rays to separate fat, lean tissue, and bone. Costs $50 to $200 per scan.

The Research: Why BMI Fails at the Individual Level

A 2026 study published in Frontiers in Endocrinology analyzed data from 12,604 adults in Chile and Mexico. The findings were stark:

  • BMI showed high specificity (96.9%) but low sensitivity (46.3%). It failed to identify 53.7% of people who had excess body fat according to BIA measurements.
  • Agreement between BMI and BIA was only moderate (Cohen's kappa = 0.443).
  • Misclassification was worse in women. Normal-weight obesity (normal BMI but high body fat) was found in 16.2% of women compared to 4.6% of men.
  • Among men with elevated BMI (25 or above), 42.8% actually had normal body fat levels. These are the muscular individuals that BMI flags as overweight or obese when they are anything but.

A separate 2026 study published in Nutrients (MDPI) examined 285 young adults and found that 12.3% had normal BMI but elevated body fat percentage. BMI-based screening completely missed them.

A 2026 review in the journal Medical Sciences evaluated 12,874 young healthcare professionals and found that agreement between BMI and visceral fat measurement was only fair (kappa = 0.29). BMI substantially underestimated central and visceral adiposity, which is the type most strongly linked to cardiovascular disease and type 2 diabetes.

When BMI Works and When It Does Not

BMI works reasonably well for:

  • Large population studies where individual variation averages out
  • People with average muscle mass and average body composition
  • Quick screening in clinical settings where time is limited

BMI fails for:

  • Athletes and regular strength trainers with above-average muscle mass
  • Older adults who have lost muscle mass (BMI may read normal while body fat is high)
  • People with normal weight obesity (normal BMI, high body fat)
  • Individuals at the extremes of height or frame size

Which Metric Should You Track?

If you are generally sedentary and do not strength train, BMI is a reasonable starting point. It will flag most cases of excess weight in that population.

If you train regularly, carry above-average muscle, or are approaching middle age, body fat percentage is a better metric. It distinguishes between fat and lean mass, which BMI cannot do.

For a complete picture, track both alongside waist circumference. The waist-to-height ratio (keep your waist under half your height) is actually a stronger predictor of cardiovascular risk than either BMI or body fat percentage alone, according to the UK Biobank study published in BMJ Open.

Real-World Scenarios

The Strength Athlete

Tom is 32, 5'10", and weighs 215 pounds. His BMI is 30.8, which classifies him as obese. He squats 405 pounds, has visible abdominal definition, and his last blood panel showed a fasting glucose of 82 mg/dL and triglycerides of 54 mg/dL. His body fat percentage, measured by DXA, is 14%.

The Normal-Weight Obesity Case

Jennifer is 38, 5'6", and weighs 135 pounds. Her BMI is 21.8, comfortably in the normal range. She does not exercise. Her body fat percentage, measured by BIA, is 33%.
